Diagnostic Challenges in Endoscopy


A selection of articles on common diagnostic challenges in endoscopy.
 

This collection comprises five articles related to diagnostic challenges commonly faced by endoscopists. In recent years, there has been increasing interest in the use of artificial intelligence (AI) for the detection of adenomas. One article highlights a potential benefit of using AI for adenoma detection – it can potentially help to maintain adenoma detection rates throughout the day. In certain subsets of patients presenting with gastrointestinal (GI) bleed, diagnosis can be difficult. A case in point, there is insufficient data to suggest the safety of video capsule endoscopy (VCE) use in patients after bariatric surgery. One article reports their experience performing VCE after bariatric surgery. In patients presenting with overt obscure GI bleeding, the interpretation and analysis of capsule endoscopy results require time and effort. One article describes a pilot point-of-care study where trained nurses provide real-time interpretation of the capsule endoscopy results. For patients with nonvariceal upper GI bleeding, there exists a multitude of scoring systems to aid physicians in risk stratification. One article reinforces the importance of validating these scoring systems in the local population. Finally, for patients presenting with gallbladder polyps, the current standard of care includes imaging to identify suspicious features suggestive of malignancy. One article suggests that probe-based confocal laser endomicroscopy can be potentially used to differentiate benign from malignant gallbladder polyps. Overall, this collection presents a diverse array of articles on common diagnostic challenges encountered in endoscopy.
